Choice Dental Plan (CDP) is designed to offer dental care to our existing patients, new patients and retirees who no longer have insurance or cannot afford dental coverage.

CDP allows patients to receive quality dental care with individualized attention to help maintain oral health and receive routine check ups and cleaning.

As part of the plan benefits any additional treatment required can be provided at a 15-20% savings.

Our professional and friendly staff can help you with your dental needs. This plan is only valid at West Campus Dental Center in Federal Way.

Gum disease has been linked to health problems like: Heart disease, Diabetes and Stroke. Regular dental cleanings and good brushing and flossing habits can help prevent gum disease.

Patients agree that West Campus Dental Center fees stated must be paid at the time of services rendered. Any service not paid for at the time of service will be billed at our usual & customary fees. Plan fees are only valid when paid at the time of enrollment.

All family members must reside in the same household to be a part of this plan. This is not an insurance product. Plan cannot be combined with other insurance plans or promotions. THIS PLAN IS NOT TRANSFERABLE. Benefit year ends 12 months from date the plan is purchased. Restrictions apply, call for details.
